One of the most common ailments is the problem that originates in the lumbar spine, i.e. the lower part of the spine, which if not treated in time can cause the well-known:
1. Lumbar disc herniation
A herniated disc is a bulging or herniation of the soft inner part (nucleus pulposus) of the disc through its outer fibrous part (annulus), which causes the nerve root to become irritated, causing considerable pain.
Herniated discs are not always caused by a bad twist, a twist, an accident, continuous repetition of movement... the truth is that most discs weaken over time, due to repeated microtrauma that leads them, in the end, to herniate. Another name for herniated disc is ruptured disc, bulging disc or nerve impingement, and sciatica is the most common symptom of a lumbar disc herniation.
2. Lumbar canal stenosis
It is another condition that causes sciatica due to narrowing of the spinal canal, i.e. spinal stenosis. We would say that stenosis of the lumbar canal is the natural ageing that affects the greatest number of adults over 60 years of age. The factors that affect it:
- Hypertrophic facet joints.
- Soft tissue hypertrophy.
- A bulging disc that presses on the nerve roots where they exit the spine, causing sciatic pain.
3. Dyscarthrosis
Although disc degeneration is a natural process of ageing, for some people one or more degenerated discs irritating a nerve root in the lower back can also cause sciatica. Disc osteoarthritis is diagnosed when excessive micro-movements at that level of the spine of a weakened disc exist or are detected causing inflammatory proteins within the disc to become exposed and irritate the area (including the nerve roots). Despite its name, this is not really a disease, but a natural part of ageing.
4. Isthmic spondylolisthesis
This condition occurs when we overload and produce a small fracture that slides one vertebra forward over another vertebra (for example, the L4 vertebra slips over the L5 vertebra). The combination of the collapse of the intervertebral space and the fracture and forward slippage of the vertebra can result in pinching of the nerve where it exits the spine, causing sciatica.
5. Piriformis syndrome
Sciatica is when the sciatic nerve is irritated as it passes under the piriformis muscle at the buttock. If the piriformis muscle irritates or pinches a nerve root that the sciatic nerve passes through, it can cause a pain that is similar to and very uncomfortable as sciatica. This is not a true radiculopathy (the clinical definition of sciatica), but leg pain can feel just like sciatica caused by nerve irritation. Irritation of the sacroiliac joint (located in the lower spine) can also irritate the L5 nerve above the sacroiliac joint, causing pain similar to sciatica. Leg pain can feel the same as sciatica caused by nerve irritation.
